Re-Entry Process Following Academic Suspension
- Complete the Academic Suspension Re-Entry Agreement form below.
- Meet with the assigned advisor for your major. The Suspension Re-Entry form must be completed prior to the appointment.
- Make arrangements for payment with the Student Accounts Office (Jefferson Hall, J-112). Contact Financial Aid if needed.
- Attend class and follow-up with any recommended referrals/resources.

Financial Aid Suspension
Once the Satisfactory Academic Progress (SAP) Committee has reviewed and approved your appeal for the semester, the appeal will be approved pending the submission of an Individual College Academic Plan (ICAP) for your major. You will need to meet with your Academic Advisor to create your ICAP.
Advisors only create ICAPs by appointment, so you must contact your assigned Academic Advisor to schedule an appointment.
Please have your advisor email a copy of your ICAP to [email protected] as soon as possible. Financial aid cannot be awarded until your ICAP is received.
Failure to adhere to your ICAP will result in suspension of financial aid. You must successfully complete the exact courses for the exact semesters specified on the plan, unless the plan leaves room for you to select from a few suggested courses per semester.
If you should need to take different courses in a different order than specified, you must request a revision in writing to the financial aid office prior to the 100% refund date(s) of the semester in question. Some revisions may require a new ICAP. Only required coursework for your program at FCC is eligible for federal and/or state financial aid.
